Skeptics and Advocates: A Safety II Discussion
DescriptionSince the start of the modern patient safety movement, there has been a lot of focus on investigating adverse patient safety events with a heavy emphasis on uncovering the causes that resulted in the adverse outcome. In this approach, the focus is to reduce the number of things that go wrong with many organizations setting a goal of Zero Harm. However, there has been a growing movement from safety that suggests the focus should shift to studying what goes right and striving then to optimize the number of things that go right. These two approaches have come to be known as Safety I and Safety II (Hollnagel, Wears, & Braithwaite, 2015) and have spawned arduous support while also garnering several skeptics. This panel consists of advocates and skeptics of Safety II and will foster lively discussion of these opposing views.
